Research Study on Expression and Interpretation of Emotional Events

Consent Form for Prescreening Process

Thank you for your interest in our study.

The study is being conducted by Dr. Matthew S. Shane from University of Ontario Institute of Technology (UOIT), and focuses on investigating the way that people express and interpret emotional events.

We are looking for healthy adult men and women who would be willing to describe both happy and sad emotional events from their life on camera within the Clinical Affective Neuroscience Laboratory (CANdiLab) at UOIT. The study has been approved by the UOIT Research Ethics Board, and has been assigned REB #14-128.

First Step: Do you qualify?

Each CANdiLab research study has its own unique requirements. If you click “agree” below, you will be asked a series of questions to determine if you qualify for this study. You are not required to answer any question you don’t want to, however eligibility can only be determined if you answer every question as accurately and honestly as possible. By clicking “agree” below and answering these questions, you are giving permission to the research staff to determine eligibility from the information you provide.

What happens next?

You are currently only answering enough questions for us to determine whether you qualify for the study. If you do, you will be contacted within 5 business days by a CANdiLab staff member, who will schedule you for a time to come into the laboratory to continue the remainder of the study. The lab is conveniently located at the corner of King and Simcoe in downtown Oshawa. The staff member that contacts you will be able to provide additional details regarding the study, and answer any questions that you may have. When you arrive at the laboratory, you will be given a more complete consent form, which will explain all aspects of the study, and obtain your signature of consent for the study procedures.

What do we do with your information? How do we keep it secure?

  1. All information collected on this website will be used only for the purpose of determining your eligibility for CANdiLab research projects. The database is SSH-encrypted, password-protected, and accessible only by authorized staff.
  2. All information provided in this prescreen will be deleted within 30 days, unless you agree to participate in the full research study. If you do agree to participate, your information will be retained, but treated with the same care that we use with all study data.

What are the risks of being in this study?

Loss of confidentiality: Your study data will be stored on secure, password-protected computers, or in locked file cabinets within the laboratory. Moreover, it will only be utilized by research personnel affiliated with this study. We will protect your confidentiality to the best of our ability; however, we do want you to know that there is always a small chance that confidentiality could be lost, and that your study data could spread more broadly.
